CIPs are responsible for maintaining the source documentation that supports their CE claims. In order to be accepted as credit, all CE documentation must include the individual’s name printed on the document and:
- Date
- Program title
- Sponsor name
- Number of hours attended
If the CE is met through a college course, the formal transcript must be available for verification.
For CE events where accreditation is claimed, CIPs must verify that the events are eligible as accredited CE hours. CIPs should not assume that a CE event continues to carry accredited CE hours just because it was eligible for accredited CE hours in the past.
A suggested format for submitting summary information of CE activities is included here. Applicants should carefully review their CE records and only submit relevant and legible information that clearly demonstrates how the candidate has met the CIP CE requirements.
CIPs are responsible for retaining source documentation (certificates, letters of attendance, etc.) for at least three years following recertification by CE. Individuals should be prepared to submit copies of source material, when requested, for audit and continuing quality improvement purposes.
Audits of recertification by CE may be conducted for verification and continuous quality improvement purposes by the CCIP. If an audit reveals insufficient credits, denial of certification is possible.
